Transition Services

Transition services are individualized activities for students who are working towards post-school outcomes. These activities promote movement in the following areas: post secondary education and vocational training, employment, independent adult living, community connections, and self-advocacy. The activities are developed based on the student's preferences, interests, strengths, and needs.

All students and families will at some point want information on the transition process. Transition planning may include obtaining knowledge on: legal information, future educational supports and opportunities, available community agencies, and employment trends-just to name a few.

Key Transition Services in Eaton County include:

  • Transition Services staff is available to provide resource information to parents, students, and educators.
  • Vocational training opportunities are available such as Career Preparation Center, School Transition to Work Program, Supported Employment Program, and Community Work Assessment Program.
  • We have partnerships with community agencies such as Michigan Rehabilitation Services, Michigan Commission for the Blind, Community Mental Health, Office of Disability Support Services, Peckham Inc., and many more.
  • Curriculum, resource, and educational materials are maintained in the Transition Services Lending Library for educators and parents.
  • Support and training for transition IEPs is provided.
